Wordpress Theme

Need a custom theme built for Wordpress blog. Have a mockup of layout. There are several themes that are very close and CSS could be applied, otherwise there is a feature I would like that may need to be hardcoded. There will be no flash (at least for now-and at most will be very limited if implemented in future) or adwords. Possible Twitter widget. No logo design needed. I have photos that will be used in banner. No other graphics needed. Basic design consists of top menu of several buttons, banner (would like revolving banner image), two sidebars and footer. First sidebar will run horizontally underneath the banner. Second sidebar and footer will act as columns underneath. I can handle finding host, uploading files, testing and importing materials from old blog which is currently hosted on WP (new site is self hosted WP) Code should withstand upgrades from WP software. Please provide examples of sites, estimate time to complete and project hours/cost. Open to using other code as well as this could probably be done in many ways as long as it is compatible with WP, server host and common browsers.
