Survey/Assessment Tool with Automated Analysis Based Off Survey Answers (Multiple Choice)

I am looking for somebody who can build an assessment tool that has 40 multiple choice questions (A,B,C,D,E) and based on the surveyor's answers, will provide a custom analysis. Each question has a category and subcategory assigned to it. Each multiple choice (A,B,C,D,E) has a point value assigned to it. I provide the analysis text for each answer.
