I need someone who is familiar with the PHP development, we already have a system in place and we just wanted to have an add-on feature. We will give you the database for our users so you can use the access and simply re-use the same credentials. This is intended for schools, the features are: 1. Create an E-Form for a sign-up sheet for volunteers 2. The E-Form creator is the school admin 3. The volunteers are the parents and teachers 4. The Event samples are school parties, picnic, concerts and any school activities that needs volunteers/helpers. 5. The E-Form sheets will contain -Even name, Date/Time, Number of Volunteers needed, Tasks of each volunteer 6. The E-Forms will be sent to all volunteers (parents), those volunteers can be grouped into Grade Levels or Classes (these criteria is part of the Database) I have here the sample site that will be the reference for this project, this is not copying the whole site but only a reference. Hello sir or madam, I have read your description and I want some clarification regarding it 1. Do you want an admit panel where your school admin can generate a new page(eForm) which will be sent to volunteers for the sign up? or 2. Do you want us to generate that page with fixed fields? 1.
