Create questionnaire script for website

I am creating a Wix website to provide information about probate in England. I am not a professional in this field, but I am experienced via life experiences. Neither am I a professional in the IT field. I have tried looking for open source Javascript to embed but have admitted defeat! Probate is the legal authority to deal with a person’s financial affairs and personal property when they die. The website it intended to advise people about the probate process which is often over complicated by profit making organisations. I wish for the content to provide support and to inspire confidence in users to obtain probate without paying high costs for a solicitor. I would like to create a free membership option that allows 6 months unlimited access to member exclusive information, a forum and resources. However, I would also like visitors and users to have the ability to make donations that would be used to help maintain and develop the site. Essentially, I would like members to be able to complete a questionnaire which when submitted returns a personalised action plan and advice tailored to the answers provided. I would like the action plan to include relevant weblinks and along with the action plan I would like the script to return any templates (letters and forms) required to complete the actions in the personalised action plan. A progress bar maybe an option and I would like to discuss the value of auto populating the template fields. I have completed a draft excel document to help understand the questions and the choice of answers. I have also identified the actions (to be included in the action plan to be completed by the user) and the templates relevant to the answers chosen that I would like produced alongside the personalised action plan. I have tried looking for open source Javascript to embed but have admitted defeat! Ideally, I would like this completed by 31.01.23 although I am flexible. I really see this as an opportunity to develop a professional relationship with a contractor that I can build trust with to help me develop my vision for the website and future associated services. Therefore, I would really appreciate an opportunity to have a discussion to discuss my ideas and to be given advice from a professional about how best to achieve my aims.
