4 hours of work... Easily done with cron job and two scripts one for editing the template and one for the cron to check the current date against the one in db and send the mail.

I imagine the script would be run as a scheduled task to scan the database records daily and trigger email messages where they match. The email template could be configured as a stand-alone file with variables to customize the message based on the matched db record, which would let you easily customize the template without getting into the main code.
