Online form linked to customer database(repost)

I have created a web site in which I have an customer area, when customer enters this area with a username and password, they will see an order form. There will be a list of items on the form? with a free text box for quanity next to it. In essence the customer will place a number by the item as to how many of each items they require. What I want is the form to recognise the name and email address of the client logged in and to show those details on the page automatically. So it will say Welcome <name of company> We have your email address as <company email address> Please order your items below. Name of item? Quantity Field Name of item Quantity Field and so on. The form will have some sub headings in it to seperate certain parts of the order form. When the client clicks the submit button, a copy is sent to the client and a copy is sent to the customer. A thank you page will appear confirming order and who it was made by. I want the form to look very presentable and will need to fit into the current web site I am developing. Obviously I would need to manage the customers details in a database together with username and passwords, so a user friendly method would be preferred. So to recap, I need a small databse, a form and thankyou page that will run on any web site with the ability to send confirmation emails to customer and my client as an order. I need to be able to add, view and amend customer details as well as provide username and password. As an addon if there is some way of recording all orders in the same database and being able to view, amend and complete orders that would be a welcome next stage which can completed at the same time. Time is of the essence a previous coder has let me down. ## Deliverables I am open to any suggestions as long as the small system works efficiently
