Mortgage Application Form

Winning bidder will be asked create a form very much like the one at [login to view URL] with a few different fields. I have attached the Word form I am currently emailing to my clients. I would also like to have the paper form redone if bidder has any experience in that. It can be in any format that can be filled out on the computer (i.e. word, excel, other) and emailed back or printed out and faxed preferably with fields that match the internet form. This form may be designed as part of a page or as a popup, depending on the format that comes out. The internet form will then be submitted as both a formatted email with the application as well as to a database. The next step in the overall project which I will also list on [login to view URL] will be the development of said database using mySQL. This should have an admin section as well as a login from information on the application that will allow a client who has applied to log on and check their status. I will also be needing another form done, as well as a renovation of the entire site [login to view URL] as well. I mention these other future projects so as to ease the design of this first form. Winning bidder should have sample forms and sample databases if possible. This project is only the basic form, but would like to hear your ideas on the database as well as the web re-design if you have any. Each seperate task will be listed on getafreelancer.com.
