PHP form to Word documents

We need someone to build a custom PHP form (consisting of 12 fields), the entered information of which must be stored in our database and on the basis of that 5 Word documents are filled in according to the corresponding values. I.e. Company Name = all instances of (company name) are replaced in the documents. Name = all instances of (name) are replaced. There must be a user panel and an admin panel. The user panel gives the option for registered users (the Php form also registers email and password) to pay for the files. They are taken to [login to view URL], so the user is prompted to pay via cards and then brought back to download the edited word files in a zip, in the same user control panel. After the user has paid, his status becomes “paid’ so he can always acquire the documents from his control panel. The admin panel allows us to see all of the registered users, their entered info and word document outputs (in zip files) and the possibility to delete them. We need someone to build this in MAXIMUM 2 days. If you can’t do it in 2 days, then please DO NOT contact us. We are trying to find someone really good who can create more forms like this in the next 2 weeks, so we are looking for a long-term partner, and we can pay the same amount every time. We need someone who is reliable and can deliver on time.
