Advanced Poll

So what im looking for is pretty much an advanced poll. Instead of a poll though, i want a scale (Attached are 2 scales that i want it to look like. choose either but make them professional looking). on each end of the scales, i want room for 2 images i can put there with a text box under them stating what the pics are. so both start out as 50%/50% and as people start to vote(and or course they can only vote once), the scale begins to tilt whatever way there are more votes. i would like to see how many people voted on each side.. and the percentage of each. i don't exactly know how this is going to work, but i would also like a comment system under each scale. i have a login system made in php/mysql and the members will be the ones using all this. so i can make the comment system in php but it has to be connected in the database with the right scale. if by any chance an admin scale can be made where one of the admins can actually put a title on the scale and upload the 2 pictures and the title of the 2 pictures then that would be great. again, i need to be able to duplicate this scale and use it more then once but keep the old ones to, as well as to contain all this info in a database.
