53133 LEADS AFFILIATE SCRIPT

I would like an affiliate script that will allow me to record LEADS to database for mortgage, merchant accounts, insurance. There will be 3 sections to the script: public side, admin panel, affiliate panel PUBLIC SIDE: there will be a form for the public party to fill out: first they would select the type of information they are intested in: Mortgage, Merchant Acct, or Insurance. Once a selection is made, the remainder of the form will populate accordingly. Merchant will have these fields: loan type: new mortgage, refinance mortgage, equity loans, or consolidate bills name address home phone work phone email ----- mortgage balance income rate your credit property type home value loan amount current interst rate fixed or adjustable merchant account will have these fields name address home phone work phone email ------- business name business website product or service sold monthly processing Insurance will have these fields type: health or life or both name address home phone work phone email ------- personal, family or business coverage Once the information is submitted to the database, the visitor will see a confirmation page. The information can all be stored at the same database, but will need to be exported by category. FOr instance, i need to be able to select dates and mortgage, and have just that information exported to a text file. ADMIN PANEL: Here, the admin can log in and view/edit/delete the affiliates passwords approve accounts commissions (commission structure will have to be flexible. depending on the type of lead, the commission will be different) I will need to be able to mark commsions as "paid" when i pay the affilaites and have it ZEROed out for the new week. There will need to be a real time stats/ hit counter and a search engine by name, date, type of lead, which can seach and export the data to a text file. also, an affiliate mass email option. AFFILIATE PANEL: Here, the affiliates can log in and view there pending commissions, hit stats, and number/type of leads produced.
