Trackable Newsletter Campaign - Recommend to a friend

Hello, I am looking to develop a php mailer script that would increase my mailing list - very simple this is how it should work - I need a form that a user fills in with the following fields Name Email Name of friend1 Email of friend1 Name of friend2 Email of friend2 Name of friend3 Email of friend3 Once the user provides the 3 email address of his friends, his friends should be getting an email automatically mentioning that a friend recommended them and they need to confirm approval. In addition they would also have the option to recommend 3 more friends and enter a prize draw. Few important notes - 1. This must be trackable, so I need to see a list of all the users who recommended their friends, but also show how many of their 3 friends confirmed approval so maybe show a table with a the list of users who recommended plus how many friends authorized the approval. 2. Once the user recommended his friends, he will be directed to a thank you page. 3. a list of all confirmed users , so I can add them to my db. 3. I will provide the newsletter template for this.
