Hello, I seen your project description but this is not giving me the full idea but i think you need a dataentry screen and this will store all information in the database and take a printout of this data. if it is like this i have a software i developed long back only some small changes is required for that.

This is not for software, but a webbased app. It is easy to use, and i can have it run off a cd.
