Custom CredentialProvider for Windows 2012 and Above

We are in process of developing a custom credential provider for windows server 2012 and above. We have developed a credential provider that takes username/password and additional authentication factor from the user, calls our APIs for validation of second authentication factor, validates password and then allows/denies logon accordingly. The developed credential provider works perfectly fine for local windows accounts but it's not working for AD accounts. We need to modify it to work with AD accounts as well. Also, we need to disable the default password provider from logon screen so that only our custom credential provider is displayed. Disabling the default password provider should not disable it from appearing at CredUI prompts.
