I want such a plugin to be build for wordpress, that whenever any user fill ups a appointment form at front-end, its data should be sent through sms to the administrator and user also. Also an alert sms should be sent to the administrator and user, before 1 hr of the scheduled appointment. I also require to maintain database at admin panel, where details of all appointments under appointment form filled will be displayed.

Select Dept. :- Dropdown option with around 7-8 departments
Your Name:- Text field
Your mob no:- numerical field
Date of Appointment:- Date picker with future present dates, no past date selection.
