Contractor Services Website/Database built

I need a global services website/database created in which contractors will pay to have their company/service listed on the website. The contractor needs to be able to enter the following (Those experienced in creating this type of website will likely have a general template of this type of website/database):

First Name

Last Name

Title

Cell Number

Business Number

Company Address

Website link (this link should open their website in a window but not direct them away from the original site)

City

State

Country

Certifications/Designations

More information (This will be a link that opens a page displaying the contractors information plus a longer paragraph of information about their company services and will show a google map of their office location)

The contractor will then pay (paypal and credit card) for their information to be uploaded and displayed on the website.

Users on the website will then be able to search first for the contractor by (not sure if service should be last):

1)service

2) country

3) state/province etc

4) city

I need to have the source code for future adjustments/additions that may need to be made in case original company is no longer around.
