Write an Android application

We require a simple mobile app, here are the features that we require:
Check user network service (online- offline) if online go to login page (credentials checked in mysql db on our server)
Get user phone number GPS location and save it to mysql db in every login.
User will fulfil the simple form and take a picture in low res (for minimum data drain 800x600 px )
and click save
picture taken in phone camera ( only camera! no other option like from gallery) and
data entered by user and gps location will add to mysql .
Application will show the user a notification.
This notification includes an open form that user must be fulfil.
if user clicks the notification app will lounch and show in main screen open forms.
notification data will frequently check from mysql db.
Important notes:
We will require the source code. Application must be coded using
JS, PHP, HTML5, XML, JSON, CSS, AJAX, JQuery. We will not accept any other language.
We require code block description notes inside the code in English
MySql connection information must be encoded in apk file.
