WordPress Forms & Twilio SMS Customizations

I have a directory website, Vantage theme from AppThemes.com. I've integrated Twilio to send the vendor an SMS if someone completes the contact form on their listing page. I have a few edits and a few add-ons I need help with. I'm pretty comfortable with writing HTML(5) and CSS(3). I need help with everything else.

1. The "Contact Listing Owner" form opens in a modal window. I need the form moved to the sidebar and visible.

2. I need to remove unnecessary fields. (email) (website) (description).

3. I need to add necessary fields. (photo upload) (location).

4. I need to add file upload (single image) to the form.

5. I need to repair & enhance the Twilio integration.

- Form entry sends two text messages. I need to condense the output to a single text message.

- I need to include the photo (#4) in the sms and send as MMS message.

6. I need to capture end users physical location (coordinates) either desktop or mobile and include a google maps (?) link of the coordinates within the the MMS message.

7. I need to store (archive) form entries for each vendor.

8. I need to display the form entries in vendors dashboard.

9. I need to add extra fields to entry for vendor management (3 text fields).

10. I need to block public access to vendor dashboards. Only vendors and wp-admins have access.
