Android App - Camera Upload to Webserver

I need a very simple app that will allow the user to upload a picture to a webserver. This doesn't have to look pretty and the user interface is unimportant as long as it functions and is easy enough for me to use. I think this should be very quick for someone who has done this before.
User opens app. Camera functionality is activated. If the user snaps a picture it is uploaded to specific location on a webserver (like [login to view URL]). No other functionality is needed.
The app should be built for Android and the webserver is LAMP. I will need a way to download the app onto a test phone and install. I will also need all underlying code and instructions for changing the webserver location.
