Copy Search Functionality from existing Site

Need functionality for search FORM on DROP DOWN to be copied from one site to another site.
New site uses different CSS, so that needs to be kept the same. Files will also need to be changed from .html to .php
What is needed.
Copy to site (Image 2)
1. Add Drop Down Option with 50 States
2. When number is entered, it routes user to /search/[url removed, login to view] and displays # entered and State entered (from drop-down)
3. Display text in form that is transparent (it's currently static and user needs to delete text)
All PHP files for existing site can be copied from - you are simply copying the backend functionality of the forms to the new site, while maintaining look and feel. Feel free to PM me for questions.
