autocomplete Combobox sql report

I need a simple web report that takes values from several autocompleting combo boxes and regular text boxes and submits that data to a sql stored procedure. The results of the stored procedure are then displayed on the page. The data to be selected from the combo boxes is generated by a sql query and only values from that query can be submitted to the SP. The code to generate the autocompleting combobox data is as follows: select code from Ingredients where (Code > N'99') order by Code the regular boxes should only except numeric data there should be 9 comboboxes that send their data to the SP as (at)items1-(at)items9 the nine regular box their data to the SP as (at)amnt1-(at)amnt9 The form should look something like this COMBOBOX1 ((at)items1) RegularBOX1 ((at)amnt1) COMBOBOX2 ((at)items2) RegularBOX2 ((at)amnt1) | | COMBOBOX9 ((at)items9) RegularBOX9 ((at)amnt1) (button to send data to SP) After the button is pushed the data should be sent to a SP named VOC with the variables listed above, the SP will then return the following fields: TotalCuppm, TotalMoppm, TotalHAP, TotalVOC, Density Those labels and fields should be diplayed below the Button (there will be only 1 row of data returned)

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):

a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
