Word label and editable fields

Hello!
I’ve a standard label that I’ve to customize depending on the customer that I’m visiting. There are basically two fields that I need to change on a customer base. Each clients gets a sheet with 35 equal labels on it.

I need to change the variables highlighted on file 2 Highlighted [login to view URL]
What I want to be done is:
1. To have the attached word file (3 Label [login to view URL]) with one copy of the label in each of the label empty spots
2. To have one single field changed and all other 34 replicated with the same data (so I don’t have no manually enter each one of them)
3. The variables should be formatted as follows:
- Field 1: font ‘Myriad Pro’, bold, italic, size 7, gray (RGB, red 166, green 166, blue 166)
- Field 2 (date): font ‘Myriad Pro’, size 6, gray (RGB, red 166, green 166, blue 166)
Preferred method is using Word’s standard label printing.
