shopify medical patient history app
the idea is to create an app to help shopify users to keep a patient medical history in the same system and avoid the use of another system.
two form templates should be included one for hospitals or clinics and the other optical store
and the user can add custom fields to the database.
should have an option to print the empty form created with the customs fields and to print the history of the patients.
should manage the doctors database to add to the patient history.
some forms examples added.
