I want to customise the look of a MailChimp form that is embedded on my website.

I want to form to have the fields on horizontal Brown, not stacked in one column. Please send example screenshots of where you have done this before. I will send you the original MailChimp code for you to customise. The final form should look like the attached image. I will require the custom HTML to be sent to me via a word document, ready to cut and paste into my website.
