I have a simple html page with a form that calculates Body Surface Area and Medication Doses. For this job, design a separate page (in html and javascript) or (for wordpress+javascript), that puts all the information into a printer-friendly plain-text 1-page layout, with added room to write in a medicine name, and a patient name.

Also give me code so I can add a PRINT button onto my BSA calculator, that will launch the printer-friendly page and pass the calculated results as parameters to it.

I *could* do this myself, but I have no free time for the next few days

I expect the usual 8 to 10 initial bidders that are automated bots, which I generally ignore. I only hire solo coders, and you must say something in your reply that tells me you looked at my page. I need it done tonight or tomorrow morning. I want to upload the final result in about 30 hours from now. Thank-you for considering.
