Your task is to center a modal on screen and also to make it responsive so that it scales properly. Also when the width and height attribute is set it would be centered and scale properly. Apart from that certain labels are appearing above form-control rather than at the left hand side of the form-control. Also there is table which I would like to make it scrollable when items are added. Kindly take note that new form-control would be added on the attached mockup. This is a simple task and you should work via teamviewer.
