I need a designed application form to order our products.
The application form should look like this URL : https://registration.smtp.com/ (but more design)
Then once selected, the application should :
1) open a back end URL and wait for the result of the URL opened to display a confirmation on the front end (client)
2) send an email

The development should be done in PHP and the list of products should come from a Mysql database.
Include a possibility to add credit card epayments in futures.
The goal is to provide a few code to add on our resellers web site. Then their clients will be able to order our products.

Is using PHP mandatory? Would you be willing to take submissions in Ruby (Sinatra Framework) or Python (Flask framework) or Node.js (Express Framework)? The database will still be MySQL.
