A company in business of providing Health Care Insurance, currently covers the states of New York, New Jersey, Connecticut, and Pennsylvania, wants to expand its reach for the customers of Delaware, Baltimore, Virginia and Washington DC. The company plans to start their operations from in the new states from 1 st January 2023. The availability of the coverage will be advertised by the company in newspapers, TV Ads, social media, and flyers to be distributed locally. The Ads will provide the website link where the prospective customers from the new states can login to get the information about the plans and if willing can register to get the coverage of the plan from this company. The prospective patients can register up to 11.59 pm on 12/31/2022 for the coverage starting at 12.01 am on 1 st January 2023. Registration will be considered complete and valid only if the patient has completed the application form with the information required, submitted, and received the message online that ‘ the application has been accepted for consideration’. The company provides the plan for : 1. Single person 2. Couple 3. Couple plus two dependent children ( this plan is applicable in case of a single parent also; the plan fee will still be the same. 4. Couple plus three or more dependent children 5. The plan for Eye Care and Dental coverage is not included in the basic coverage. The customer has the option to enroll for Eye Care plan and Dental coverage with an extra fee, the customer can enroll for both , one or none. 6. The applicable fee for the basic coverage , the plan for Eye Care and Dental coverage will be visible to the patient after finishing that specific part of the application ( for example after the customer completes the portion on ‘Basic Coverage’, the website will display the monthly fee for the coverage. Similarly on completing the portion on ‘Eye Care’ and ‘Dental Plan’, the website will display the monthly fee. 7. The customer on completing the application will have to enroll for online payment of the monthly fee. The company needs to develop the online application form for the registration by the customers in the new states where the coverage will be provided. The new online application is required because the company does not offer Eye Care and Dental Plan in the states where the plan is provided currently. The registration will start on 12.01 am on 1 st November 2022. To register the customer will be asked information on the following and will be required to fill the application accordingly: a). Date of the application b). Full Name c) If the application is for self or other people to be included for coverage. d). Full Name and relationship with the other person / persons included in the application for coverage. e). Date of Birth of all the persons included in the application f). Social Security Number of all the persons included in the application g). Identification of all the persons included in the application h). Present address of all the persons included in the application i). Monthly family income j). Choice of the Basic Coverage Plan ( self or with family members ) k). After the step ( j ) the customer will be prompted to ‘ Submit “. l). On submission as per step ( k ), the system will display the ‘ Monthly Fee’. m). The customer will accept the fee. n) The system will display the ‘Terms & Conditions’ after the acceptance of the fee by the customer and if the customer accepts the ‘Terms & Conditions’ then the application will be considered registered and accepted. p) After step ( n ) the customer will be given the option to enroll for --- i).
