Need call to action button and contact form

I added a referral program page to my website and I need the form I put on it to be made functional so people and insert information and it will forward to the proper email. I also need a call to action widget on my main page to point to the new referral page. The contact form is in the attached link I will also need a submit button on the new page and a checkbox next to the statement at the bottom. Summary: 1. Add call to action button with short message to main page (Call to action should point to referral page) 2. Complete the form on the referral page and add a submit button and a checkbox for statement 3. All spacing/ padding should be done correctly so nothing is overlapping or over extended white space (this includes mobile)
