Build me an online quiz/survey

Hi, I need a Wordpress and mobile-friendly quiz/survey that has the following details: - 21 questions, with 2 options each < provided by me - 3 Axes of answer scoring/analysis (7 questions deal with the first axis, the next 7 the second axis, the last 7 on the third axis.). This is the complex part of the survey. < provided by me - A paragraph for each of the possible scores < provided by me These are all provided in an easy to understand Excel file. I also provide Wordpress hosting and an admin account, if necessary, the site is already set up, I just need someone to come in (if need be) and code the test/survey to my exact specifications. These would be: The person sees a single page with 21 questions. Each question has an image header, the question and the two options. Once they reply to all the questions, they are taken to a results page, which shows their results on the three axes... i.e.: // start of example You finished, congratulations! graphical bar indicating an axis (i.e. liberal < ----------|--> republican) [text provided for 5 points liberal 2 points republican] graphical bar indicating the second axis (i.e. libertarian <-----|----> communitarian) [text provided for 3 points libertarian 2 points communitarian] graphical bar indicating the third axis (i.e. bla <-----|----> blabla) [text provided for 3 points bla 2 points blabla] Share your results on Facebook, Twitter, Email or Print them. // end of example The scoring has to be secret (i.e. not visible from view source), the user must be able to share his/her results to the world via social media, print them or email them. The results have to be graphical, but nothing fancy. I would appreciate, if possible, a progress bar on the survey and ajax functionality (i.e. jumping to the next question, once the first is answered, but it's not obligatory). I'll take quick and dirty, as long as it works and it looks decent. You're grading 21 questions based on 6 things (3 pairs) and displaying them on a single page in an easy to understand format. Please bear in mind that we're an NGO and don't have a lot of funds for this, but that we will credit your work as much as possible. We also need this done as soon as possible.
