Basic form convert to HTML5 CCS3 mobile compatibility - open to bidding

This form is for registering products I sell (product registration form for warranty purposes), with basic form capturing, all data is stored in mySQL database. I have an old school form, and need it converted to HTML5 with CCS3 elements so it can be viewed on mobile devices as well as desktop browsers. If it detects an older unsupported browser, then it will use the old format. Skills: 1. Basic MySQL and PHP needed 2. Must know how to use HTML5/CCS3 3. I have a selection of items that needs to be displayed in pictures (can be multi-selection checkbox), so user when selects each picture, form knows what is selected. 4. Admin section to add or remove products (products not listed will have an OTHER field where user can either upload image of item or type product model. 5. Admin section to see all registered people who filled out form. 6. Form should be downloadable in excel file (if this cannot be done, I can find someone else to do it), or if integration cost to must to do this, I can skip it. 7. Admin should have option to have user check if email is valid by sending a confirmation link to register user. If uncheck, user will not have to validate email address. Any clarifications, let me know. Note, this is a basic POST type form like inquiry page but data gets stored in database and also email is sent for user registration record.
