From excel to data base

I need a simple input - output data , report and stock( from in-out data orders) for tracking( "history") this orders ( that i will be able to print from report ) . IMPORTANT : The order must look with approximation ( like in excel file attached - they have same formulas that need to be the same " sum" and "multiply" ). An example of what i need is something like Desktop Services Template1 from Microsoft access . Can be done in scripts like xcrud, flexgrid, ajaxgrid , php ,mysql or Microsoft access . I only have a window of 6 days . I need same example to see if you are able to complete the task.
