If this was photo-manipulated, then whoever did it would've been one heck of a photoshopper with extensive knowledge of elephant anatomy. I can discuss more on my thoughts through the chat, but this looks to be a genuinely emaciated elephant. It's probably not impossible for someone to have this skill in photoshop, but it's highly unlikely this image was manipulated to this degree.
