Application Form Flyer

We are taking an online form and converting it into a Application Form Flyer at 8.5"x11". The client would like it redesigned to make the Application Form look better then currently in the PDF supplied. This is multiple pages between 9 to 10pages. Please create this Application Form Flyer in Adobe Indesign .indd This form flyer is to be used by someone filling out by hand, NOT electronically filled out so keep that in mind when creating the form field areas to be big enough so that someone can write into them. When the project is done it is expected that you package up the files and send them all back to me. I will attach a .zip file which has an example InDesign file that you can use which already has the header and footer setup for you. In this file has the swatches of color that can be used for this project which is basically a dark blue and light blue with some grey. This zip also contains the logo file and Frutiger fonts to use. I will have attached a .pdf file ([login to view URL]) which contains all the text content and form field content. Since we are converting the content from the web to print we do not need to include "* required information" and don't need to use the asterisk * On page 1 of the PDF we don't need to include the text so just disregard the following "Instructions to Applicant 1. You must fully and accurately complete the Application for Employment. Incomplete applications will not be considered. WMCHealth may use the information given in the application as part of the pre-employment screening to verify the applicant's previous employment and background. 2.
