Customizing the Google Forms -- 2

We have a Google Form hosted on a Squarespace site which we'd like to customize. We'd also like to develop some scripts for handling the data submitted into those forms. We seek an experienced Google Apps, Scripts and Studio developer to help us with this and show us what else can be done to improve our workflow.

We will provide a link to the existing page so that you can see what is live and evaluate what work will be needed.
