Registration form to web database

We need to create a very simple registration process for a baseball league. Here is how we would like it to work. Step 1. Users will fill out a copy of our fillable PDF Step 2. Once filled in they will hit submit, where it will print a copy locally, send an email copy to them and us, and then take the results and put them into our website database where it is available immediately for us. Step 3. We can then view the results on a web based admin where we can add/edit/delete results (users) and add payment details (how they paid us manually) and a comment next to it. Your job will be to 1. Turn our PDF registration form into a fillable PDF 2. Create the backend admin website (user login and password protected) 3. Have them working perfectly together. Registration form is attached for you to view. Fillable PDF Specs: Pant Size should be a drop down with sizes Youth XS to Youth XL and Adult XS to Adult XXL) Division should be dropdown list: Blastball $85 (2012) TBall $115 (2011/2010) Atom $135 (2009/2008) Mite $165 (2007/2006) Squirt $165 (2005/2004) Peewee $165 (2003/2002) Bantam $165 (2001/2000) Midget $165 (1999/1998) Player Name (first and last) should be separate boxes Every other spot is self explanatory Section for To be completed by BDMSA executive only: Fee should auto populate from the Division selected above all the rest of that box can stay unlinked and not fillable, we will manually fill it in when we print it out. Backend Admin: Should give us the option of what fields (from the form) we can display in the table view. So if we only want to see, Player last name/Player First Name/Divsion/Gender/birthdate we can view just those in the table. If we want to view more options we can check them off. We will need an extra column in the backend table view, payment type (Credit Card/Debit/Cash/Cheque/Jumpstart) this will be a dropdown so we can select how they paid us, with a Comment Field next to it for any notes we need to add regarding payment. Results will also need to be able to be exported to an excel spreadsheet. Budget for this project is $150CDN and we need it done in 3 days.
