I need to create a WordPress plug-in for calculating prediabetes risk, I need my WordPress site to have the function similar to this website:
https://doihaveprediabetes.org/prediabetes-risk-test.html

Requirement:
(1) I can translate the words to my own language
(2) Compatible with majority responsive theme
(3) Stunning design
(4) Provide back all the source code for future development
(5) Function is the same as the current online calculator (e.g. people can print the pdf or email the test results)
(6) Include other functions that you think is relevant and will make this plug-in more useful (e.g. BMI calculator etc)

* Please provide mockup for proposal so that I can choose the winner easily*
