We need an design for health insurance card (as creative as the ID sample cards attached) made with all elements separated in layers in Photoshop. The design must look like for a professional heath insurance card or a government identification. It is strongly recommended to use guilloche as background image of the card. Multiple designs might be submitted.

Feel free to arrange, color and design something cool but professional. I might end up choosing more than one winner if the design is appropriate.

You will find, into attache files, some sample cards from the web and sample design (frontside and backside) only for your reference. The logo is include into the attached files and should be used.

NOTE:
Card size: 85.5*54mm
Bleeding size: 89*57 mm.
Text on the card is in French language
